Access Control Form Overview

The Access Control Form (ACF) is used to request and change access responsibilities for employees. The form is used to:

  • View a user's current and requested responsibilities,
  • Request, change, or terminate a user's approved responsibilities, and
  • View the history of a user's responsibilities.


The ACF is available under the following responsibilities:

  • UAB HR Officer, 
  • UAB GA End User, or 
  • UAB GL End User.
